You should have received an Email from Allmeds with your User Name and Temporary Password. You will be instructed to change this password to something you can remember – you will be able to review your chart notes from Dr. Taha and send messages, so make sure you keep this information handy. Please make sure you complete both sections (The web portal will send you a message across the screen stating you have completed the patient portal). There will be sections related to the pain that you are having - please pick the one that is most appropriate.

Forms to Fill Out

Please print and fill out the 2017 Patient Packet and bring this along with your Insurance Cards and ID and all testing films (probably on a CD) to your appointment.

NOTE: If you are being seen for pain in the middle of your back (Thoracic) – print the form that is closer to your problem (if it is closer to your upper back – print neck form) (If your pain is lower in the back print the lumbar form).
